my quick take.
we measure success by the stories we tell.
I'd want everyone on the team to be collectors of stories. Stories of youth, youth workers, staff etc living into what they value, and hope for as a church.
the stories we tell are how we measure what is successful.
What we measure is what we value right?
Everytime we gather as staff, or youth , or youth workers etc, someone needs to tell a story... or more than one.
This is a bit more free form than a list of questions, but it may bring about specific you are looking for. (depending on the questions)
Of course you may be asking, for more, but this is my first take.
my apologies if it's not a fit.
What's great about stories, it that it gives leaders an opportunity to affirm or adjust what we are shooting for as community.
